• NOTICE OF CHANGE IN EMPLOYMENT QUARTERLY REPORT - Required by all funeral establishments to be submitted quarterly.

• ASSUMPTION OF RESPONSIBILITY FORM - Required by all establishments when the responsible funeral director changes.
